I agree to the following contract terms: INITIAL
1. I agree that this Dog is being adopted as a family pet. The Dog will have indoor privileges, will sleep inside the house, and will not be kept in the yard without direct supervision.

2. I agree to provide this Dog with fresh water and quality dog food.

3. I agree to provide this Dog with proper regular veterinary care which includes, but is not limited to: annual physical examinations, vaccinations as recommended by my veterinarian, rabies vaccination as required by state law, heartworm preventative, flea/tick preventative, and prompt treatment for any illness or accident.

4. This Dog has been examined by a veterinarian prior to adoption and has received all age-
appropriate vaccinations, but I understand that STEPHANIE BRYAN SIPINA is making representations or warranties regarding the Dog’s health, physical condition, breed/breed
mixture, or age and thus dog must be adopted with a health test certificate showing health test record.

5. I understand that veterinary problems may arise with this Dog, including serious illness requiring emergency care or surgery, that those problems are my sole responsibility, and that I will bear all related costs for prompt veterinary treatment.

6. I will ensure proper licensing of the Dog and will attach the appropriate license tags, rabies tag, and personal identification tag to a non-choke collar to be worn by the Dog at all times. I will ensure compliance with all applicable local and state statutes.

7. I agree that STEPHANIE BRYAN SIPINA may make a visit at a mutually convenient time or call once a year to ascertain that all of the above conditions are being satisfied. If STEPHANIE BRYAN SIPINA contacts me to inquire about the Dog, I agree to make a good faith effort to return the call or email as soon as possible.

8. In the event the Dog becomes lost, I will IMMEDIATELY notify STEPHANIE BRYAN SIPINA

9. If I can no longer keep the Dog, I will notify STEPHANIE BRYAN SIPINA IMMEDIATELY to
give them the first opportunity to take the Dog back. I must STEPHANIE BRYAN SIPINA LEAST one month’s notice to assist in relocating the Dog, during which time I will pay for
boarding charges if the Dog cannot remain in my home safely. I understand however that
STEPHANIE BRYAN SIPINA is not obligated to accept the return of my Dog.

10 I must notify STEPHANIE BRYAN SIPINA of any behavioral problems that have
. occurred at any time during ownership and I agree to pay for a professional trainer’s evaluation in case of biting or aggression issues.

11 I agree to accept responsibility and ownership of the Dog at my own risk and I release
. STEPHANIE BRYAN SIPINA and its agents from any and all liability arising out of possession and ownership of my Dog. I agree that I am assuming all liability and financial responsibility for
the Dog as of the date of this contract STEPHANIE BRYAN SIPINA and its agents will not be held responsible for any damages or expenses incurred during my ownership of the Dog.

12 This Dog’s medical record will be given to me on pickup. I understand that STEPHANIE
. has made no representation concerning the health, condition, breed/breed mixture, training, behavior, and temperament of the Dog and that the Dog is adopted “as is”.

13 I understand that the Dog’s adoption fee in REFUNDABLE.

14 I UNDERSTAND THAT FAILURE TO COMPLY WITH ANY OF THE ABOVE
. PROVISION COULD RESULT IN FORFEITURE OF THE DOG STEPHANIE BRYAN SIPINA.

15 I understand that by voluntarily signing this agreement, I am entering into a legal and
. binding contract with STEPHANIE BRYAN SIPINA.
